Providence Health & Services Care Management Manager - Social Worker in Seaside, Oregon
Providence is calling a Full-Time Care Management Manager to Providence Seaside Hospital in Seaside, OR.
In this position, you will:
Be accountable for the direction and management of the inpatient and Providence Medical Group (PMG) Care Management Departments, as well as supporting Home Health and the Palliative Care program for the North Coast.
Provide leadership and oversight for the clinical care management model, and operations of the care management department for the facility and PMG medical clinics.
Promote team functioning, advance the clinical care management model and improve coordination of patient care in collaboration with all other disciplines.
Be responsible for adherence to a budget and resource targets.
Work with the Regional Director CM, Providence Health Plan Manager of QMM Disease and Case Management, PHSOR leaders, clinical professionals, health plans and Coordinated Care Organizations, Hospital Leadership, Clinic Leadership, PMG providers, and community and government agencies as appropriate to establish effective discharge planning mechanisms and develop new models for transitioning patients back into home and community settings.
Assure timely and appropriate care coordination and case management for high-risk and at-risk patients with complex medical and psychosocial issues by applying population based disease management approaches and application of clinical protocols.
Effectively interact with Medical Staff, facility executives/leaders, Utilization Management, Finance and continuum and community partners.
Coordinate and provide oversight of use of the Patient Assistance Fund programs at the facility level.
Operate as a strategic partner to other Oregon Providence Health and Services (PH&S) facilities in meeting the Triple Aim.
Required qualifications for this position include:
Masters degree in Social Work
5 years supervisory experience, including program planning, performance management, staff development and evaluation.
5 years Care Management experience
