Providence is calling an Occupational Therapist - Home Health Services (1.0 FTE, Days) to Providence Seaside Hospital in Seaside , OR.

We are seeking an Occupational Therapist - Home Health Services who will manage and provide skilled occupational therapy to patients and clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician directed plan of treatment, taking into account individual patients special or age related needs.

In this position you will have the following responsibilities:

  • Provide occupational therapy applying therapeutic principles and practices as well as providing care appropriate to the ages of the patients

  • Communicate with patients, families, caregivers, insurers and other health care workers in a positive manner

  • Demonstrate flexibility and adaptability to adjust daily schedules to meet service area demands

  • Safeguard confidentiality for all patients, medical staff, employees, and other related information

  • Evaluate existing service and equipment and identifies options with Providence Health & Services facilities or local companies for new services

  • Plan and implement a comprehensive occupational therapy treatment plan, modifying and progressing the program in response to changing patient needs

  • Consult with other members of rehabilitation team to select therapy programs consistent with needs and capabilities of individual

  • Provide written and verbal instructions to the patients and caregivers to assist the patient in resuming life in the community or in an established safe environment

  • Identify specialized needs requiring specialized follow-up after discharge and makes the necessary referrals

  • Support and comply with the Continuous Quality Improvement program

  • Maintain accurate, thorough, and timely patient-related computer documentation in accordance with department policy and procedure

  • Design and construct special equipment for individual patients and suggests adaptation of individual's work and home environments

  • Regularly consult with department manager or supervisor regarding patient status and program through discharge

  • Provide quality patient care in an efficient, cost-effective, caring manner

  • Maintain a positive, professional personal presentation at all times

  • Maintain and enhances professional development through appropriate educational experiences

  • Enhance personal professional growth by establishing individual goals, providing in-services to others, conducting training programs, participating in community activities, and contributing to program development

  • Supervise therapy assistants, students, volunteers and other observers

Required qualifications for this position includes:

  • Bachelor's Degree -OR- Master's Degree -OR- Ph.D. from an accredited Occupational Therapy Program

  • Current Occupational Therapy licensure through the State of Oregon -OR- temporary license if new graduate awaiting licensing exam results

  • Current American Heart Association - Basic Life Support (AHA-BLS) (Basic Life Support for Health Care Provider) -OR- ACLS (Advanced Cardiac Life Support) required within 30 days of hire
